The Company: A 40-bed hospital set in a quiet residential area close to Huntingdon that focuses on enduring mental health issues: - service users have enduring mental health issues, often coupled with chronic physical health problems and are likely to present with challenges that are difficult to manage in mainstream care or in the community. They may have a forensic background and need to step-down from medium or low secure units. Occasionally an individual may be admitted following a court outcome. The Role: The role of the CPN is to be the central figure in the process towards seamless discharge/transfers or alternatively to facilitate and enhance social and family contact with those patients whose conditions prevent a further step-down move. The CPN will be responsible for discharge planning from the point of referral to visit/admission, and will become the named nurse for patients approaching discharge CPA's. The post holder will forge links with Social Workers and other CPN's within the patient's locality and to supervise the discharge process in order to achieve the discharge objectives. The CPN will liaise closely with hospital based staff especially nursing medical and advocacy services and will function as part of the MDT Team and participate in Support Time and Recovery/OT endeavours to form links with outside/voluntary Agencies. The post holder will also be expected to work as an autonomous practitioner, primarily working within the community but also in the hospital setting.
