Transition Support Worker

My client with premises based in Evesham, Worcestershire is looking to recruit a Transition Support Worker on a Permanent basis.

The main purpose of this role is to support young people leaving the care of a local authority to live in supported accommodation in the community. As part of this role you will monitor, support and build individual plans to support young people into adulthood.

Some of the responsibilities for this role include

- Liaise with colleagues, agencies and groups to identify necessary resources for the young persons pathway to adulthood
- Maintain effective records
- prepare reports
- attend meetings
- comply with all policies, advice and guidance set out
- support the supervision of lower graded staff
- Support young people to meet hoer own personal care needs
- support and encourage young people to make the right choices
- support young people to remain safe in their home and respond to safeguarding events appropriately

As a suitable candidate you will offer the following

- Suitable NVQ Level 3 or 5 qualification in Health and Social Care
- previous Experience of working with young people up to the age of 18
- Knowledge of child and adult development
- knowledge and understanding of difficulties facing young people
- excellent communication skills
- Good IT skills

The pay rate for this positions will be determined by the level of qualifications held by the suitable candidate
